  1. a Participants who enrolled in the study between July 1, 2001, and June 30, 2003, were counted among the deceased if two of the following personal identifiers were an exact match between Millennium Cohort and mortality data sources: Social Security number, last and first name, date of birth. Deceased counts are not mutually exclusive.
